Form 1583
The USPS notarized application a CMRA must collect from each customer before accepting mail on their behalf.
USPS Form 1583, "Application for Delivery of Mail Through Agent," is the federally required notarized authorization that a CMRA collects from each client. It names the recipient, two government-issued IDs, and an authorized agent. Filing Form 1583 places the address into the USPS CMRA registry and triggers downstream PMB classification. Because Laramie Ledger is not a CMRA, we do not file Form 1583 for members and members do not need to complete it for our services.
